                                    Addressing inequities in health care and outcomes requires accurate, reliable and valid data on demographics and social needs. Challenges to collecting and managing data must be addressed to build trust—both among individuals from whom data are collected and organizations tasked with using that data. The panel shares perspectives on collecting, managing and using data to understand population trends and address individual needs.Â
