Tennessee Medicaid (TennCare) believes that a strong primary care system is the backbone of a thriving healthcare delivery system. Their primary care transformation strategy emphasizes the crucial role of primary care providers in preventing illness, managing chronic conditions, coordinating care with other providers, and engaging members in the community. 
In this session, TennCare will share their model design, highlighting the importance of stakeholder engagement in the design and updates, the challenges faced, and the lessons learned. They will discuss their approach to using NCQA HEDIS measures and total cost of care to gauge success, their collaboration with Managed Care Organizations (MCOs), and recent evaluation findings.
