Existing quality measures do not effectively evaluate what matters most to individuals, especially those with complex care needs. Currently, value-based payment program measures tend to focus on disease-specific, intermediate outcomes or utilization-based outcomes. The person-centered outcome (PCO) measures work in tandem with clinical care to help people living with complex health needs make progress toward a goal that matters most to them.
Clinicians and health plans can use a person-centered outcomes approach to identify what is important to individuals and document these goals in a standardized way. This approach enables clinicians and plans to work with the individual to track and assess achievement overtime for improved patient engagement and quality measurement purposes. The PCO measures have been successfully tested in case management, serious illness programs, long-term services and supports and with behavioral health organizations. These types of quality measures have the potential to fill a critical gap in accountability and to promote person-centered care.
