Person-centered care is only possible when acknowledging the full picture of health—both the emotional and practical obstacles people encounter. The best-intentioned health plan engagement practices won’t motivate members to complete screenings, adhere to care plans or adopt healthier lifestyles if they don’t have their most basic needs understood and met.
This session highlights how UnitedHealthcare Community Plan of Massachusetts and Pyx Health use home-delivered food and social support to engage diabetic members, identify HRSNs, and drive measurable improvements in screenings, medication adherence, and quality outcomes.
