Description

How can Medicare accelerate prevention and reward measurable health improvement? Join leaders from the CMS Innovation Center to learn how two complementary models—MAHA ELEVATE and ACCESS—are advancing prevention and aligning payment with outcomes. MAHA ELEVATE funds evidence-based functional and lifestyle medicine interventions grounded in nutrition and/or physical activity to build the evidence base for chronic disease prevention. The ACCESS Model tests outcome-aligned payments by linking recurring payments to risk-adjusted improvements in conditions such as hypertension, diabetes, CKD, chronic pain, depression and anxiety.

This session will explore how these models integrate technology-supported care, promote accountability through published outcomes and generate actionable evidence on cost, utilization and patient experience. Health plans, provider organizations and quality leaders will gain insight into how prevention-focused, outcome-based strategies can complement traditional care while advancing measurable quality improvement.
