The University of Oklahoma (OU) Sooner Health Access Network (HAN) was one of the earliest community-based complex care management models in the nation and has grown into Oklahoma’s first NCQA-accredited complex care management program. Serving more than 80,000 Medicaid members—including individuals with complex medical, behavioral and social needs—the OU Sooner HAN blends evidence-based models, interdisciplinary care and person-centered engagement strategies. 

This session offers a practical, real-world look at how the HAN  model was developed and implemented in Oklahoma, how it evolved during major Medicaid reforms and how it consistently delivers measurable results. Attendees will learn the essential elements of a successful complex care management system that improves health outcomes, supports primary care practices, generates significant cost savings, and enhances member experience. Whether your organization is building a new program or strengthening an existing one, this session provides a proven roadmap.
