Visualizing, Acting, and Improving Health Equity in a Leading Safety Net Hospital
Description

Led by Dr. Yolanda Wimberly, Chief Health Equity Officer, the team at Grady Health -- a health care anchor for the Atlanta community caring for the historically underserved -- understands that delivering high-quality, equitable care is grounded in reliable data presented in a way that makes the stories in the data clear and moves people to act. This session will take the audience on a data journey through a real case study to demonstrate how the dashboards and tools reveal the story in our data and inform decisions and improvement initiatives. From a patient's initial visit through cancer screening, diagnosis, treatment, and outcomes, session participants will experience the powerful insights gained by viewing a patient's healthcare journey from their community to the health system they must navigate to receive care. The case study will also include the clinician's and support team's journey as they identify opportunities to improve patient care equitably and compassionately throughout the care continuum. During this session, the presenters will deliver critical insights about the work that Grady, in collaboration with the analytics and visualization team at Sellers Dorsey, has undertaken to engage and collaborate with clinical and operational support teams to capture, analyze, and visualize data focused on health equity initiatives overall, and for five specific cancer diagnosis. The presenters will describe the framework and techniques essential to the project's success, such as conducting discovery sessions, establishing strong data governance, prototyping solutions, soliciting feedback, and using technology to its fullest to ensure comprehension and adoption of what was created.
