Veterans are often missing from health plan strategies—not because they aren’t there, but because they’re invisible in the data. Despite being disproportionately impacted by chronic illness, mental health conditions, and unmet social needs, veteran members frequently fall through the cracks of traditional population health models. With over 18 million veterans in the U.S.—many of whom rely on both VA and non-VA care—this group represents a critical, yet often under-engaged population in Medicare Advantage, Medicaid, and ACA plans. Recent policy shifts, including the PACT Act and new Special Enrollment Periods (SEPs) for veterans, have created momentum for rethinking how health plans identify, support, and deliver value to these members. This session brings together leaders from DUOS, Humana, and PsychArmor—three organizations deeply rooted in veteran advocacy and system transformation. Together, they will share the case for embedding veteran-focused care into your quality strategy, and how doing so can drive measurable improvements in HEDIS® performance, CAHPS scores, and overall plan performance. A groundbreaking collaborative initiative between Humana and DUOS, designed to enhance care for Louisiana veterans with chronic conditions. Through this program, eligible Humana Medicare Advantage members complete a personalized assessment with DUOS to uncover unmet social and health-related needs. From there, members are matched with relevant resources—ranging from their MA plan and VA benefits to local community programs and government supports. By coordinating wraparound services and digital navigation in tandem, the program helps veterans address barriers like food insecurity, housing instability, financial stress, and social isolation—all key risk factors associated with poorer outcomes and increased suicide risk. This isn’t just about better outreach. It’s about reimagining access. Healthcare is a fundamental right, especially for those who have served. Yet, systemic barriers often prevent veterans and their families from receiving timely, equitable care. Whether it’s navigating complex benefit systems, managing multiple chronic conditions, or accessing behavioral health support, veterans require a different approach—one grounded in cultural competence, trust-building, and whole-person support.
