Diabetes and other chronic conditions continue to challenge clinicians, in part due to inconsistent access to patient-generated data—limiting the ability to deliver timely, personalized care. Diabetes management devices like continuous glucose monitors (CGMs), insulin pumps, and smart pens hold enormous potential, but without seamless integration into electronic health records (EHRs), their impact remains unrealized.
This session brings together experts in clinical care, informatics, and health system strategy to explore how integrating diabetes device data can act as a “force multiplier” for improving outcomes. Panelists will share real-world lessons on overcoming technical and clinical barriers, optimizing workflows, and aligning integration with quality measures and value-based care initiatives.
