Diana Castellanos is a recent Pomona College graduate passionate about empowering patients to manage their health. During her undergraduate career, Diana interned with Universal Community Health Center (UCHC) to learn about the unique ability of health centers to provide quality care through active listening, cultural immersion, and humility.

Diana returned to UCHC eager to contribute to the organization’s mission by providing patients with tools and education to navigate their health journey. As the Lead Remote Patient Monitoring Program (RPM) Patient Navigator she supports patients in managing their diabetes and hypertension. Diana is committed to expanding the reach of the Remote Patient Monitoring Program and enhancing patient engagement through collaborative solutions.
