Yolanda Bowes is a Senior Program Manager at Care Transformation Collaborative of Rhode Island (CTC-RI). Yolanda supports several projects that support primary care practices in implementing best practices in STI screening, implementing billing codes for CHWs, and supporting a task force formed to address the primary care workforce shortage. For the past 3 years, Yolanda led the CTC-RI team in supporting over 30 primary care practices in improving their collection and use of accurate and complete demographic patient information with a focus on race, ethnicity and language and served on a statewide workgroup to establish standards for demographic data collection.
Prior to joining CTC-RI, Yolanda managed the Community Outreach department at an FQHC in Connecticut for 14 years and was involved in primary care operations at the FQHC and a primary care practice in a large health system. In those roles Yolanda implemented and managed innovative programs that provided outreach and connections to social service programs to improve the health and well-being of patients and the community and designed and implementation of new workflows and practice improvements, training staff and participating in strategic initiatives.
Yolanda earned a bachelor’s in management from Eastern CT State University.
