Sonia Tucker is the Vice President of Population Health at San Ysidro Health (SYH), a high-performing Federally Qualified Health Center (FQHC) that provides care to over 120,000 patients annually in San Diego County. Sonia has been working in Quality and Population Health for over ten years. She is a Venezuelan physician with a master’s in business administration with a concentration in Process Improvement and Health Care Administration. She has also 20 years of experience focused on epidemiological work and implementing health promotion and quality improvement programs, specifically among underserved communities with a population health approach. She has speared the inception and operationalization of the Health Homes Program, a program of the Health Care Services of California designed to address Social Drivers of Health (SDoH) for underserved Populations in California. She has also been directly involved with projects to reduce health disparities by addressing SDoH while utilizing technology to provide appropriate resources to those in need and to use data for risk assessment and program management. Sonia has contributed to the program’s development and implementation to engage populations historically underrepresented in biomedical research, such as southeast African and southeast Asian communities. Sonia is Co-Principal Investigator of the DRES-POCAI (Diabetes Retiopahy Screening Point-of Care Artificial Intelligence) clinical trial in a primary care setting.
