Matt Gitzinger is the Executive Director of Population Management for Greater North Carolina at UNC Health Alliance, the clinically integrated network and population health services organization of UNC Health. In this enterprise leadership role, Matt drives strategic alignment across the care continuum, advancing performance in value-based care and improving the health of populations served by UNC Health’s expansive provider network.
Matt oversees a multidisciplinary team delivering high-impact programs—including embedded and virtual care management, social drivers of health (SDOH) screening and response through a team of community health workers (CHW), and clinical nutrition services. He also leads key operational partnerships with UNC Health hospitals to ensure seamless coordination of population health initiatives across inpatient and ambulatory settings.
Accountable for the execution of enterprise-wide gap closure strategies, preventive health campaigns, and quality performance optimization, Matt’s leadership ensures CIN providers succeed under complex value-based care contracts. His work enables measurable improvements in health equity, access, and clinical outcomes—especially in rural and historically underserved communities.
During the COVID-19 pandemic, Matt led the design and execution of UNC Health’s equitable vaccine distribution model, coordinating logistics across the health system and community partners to ensure reach, trust, and transparency.
