Dr. Hiatt is a clinical pharmacist with a background in community pharmacy, ACO/VBC operations, and Medicare Care Management. He currently serves as the Director of Medicare Program Operations at Community Care of North Carolina where he has built and launched an Advanced Primary Care Management partnership program to provide care management for the Medicare and Medicare Advantage populations at Primary Care Practices across the state of North Carolina. His approach to care management is shaped by his clinical work and background in Value Based Care to provide a robust clinical model with dedicated clinical teams targeted at improving chronic condition management and lowering patient utilization and cost while also providing upfront financial support to the partner practices to bolster their ability to stay viable in a challenging financial environment.
