Dr. Shaminder Gupta is an internist and Chief Medical Officer at Monogram Health, where he leads clinical strategy, quality improvement, and physician engagement to advance multispecialty health outcomes for Monogram’s complex patient population. Monogram Health is the leading national multispecialty provider of in-home, evidence-based treatment and care for patients with progressed chronic conditions. Monogram’s clinical operations employs a robust and integrated clinical team of physicians, specialists, advanced practice providers, registered nurses, pharmacists and social workers that are trained across six specialties (i.e., cardiology, endocrinology, pulmonology, nephrology, behavioral health, and palliative/end of life care) to diagnose and treat all prevalent chronic conditions present.
With over 20 years of experience, and a strong background in value-based care and population health, Dr. Gupta helped shape the deployment of Monogram’s innovative, in-home multi-specialty treatment model and a myriad of evidence-based clinical services, including complex case and disease management, utilization management, and medication therapy management, to revolutionize and transform the way health care is delivered for those with multiple chronic conditions.
Prior to coming to Monogram, Dr. Gupta worked in both private and academic practice, partnering with medical institutions such as LSU Health, Leonard J. Chabert Medical Center, and Fresenius Medical Care. Dr. Gupta began his career launching his own nephrology practice in rural Louisiana in the wake of Hurricane Katrina. In 5 years, he achieved a 40% home dialysis rate for this undeserved and vulnerable population. Dr. Gupta previously served as the Chairman of the National Kidney Foundation of Louisiana and currently co-chairs Monogram Health’s National Clinical Advisory Board.
Today, Dr. Gupta resides in the greater New Orleans area with his wife and 2 children and continues to serve as an Assistant Professor of Medicine at Tulane University, Saba University, LSU Health, and Medical University of the Americas. Dr. Gupta earned his Doctor of Medicine from Louisiana State University.
