Sonal Nanavati is the Manager of Quality - Population Health at Boston Medical Center Health System, where she leads quality strategy across MassHealth Accountable Care Organization (ACO) contracts. In her role, she partners closely with clinical, operational, and data teams across Boston Medical Center and its ACO partners to advance evidence-based quality improvement initiatives and improve outcomes in key areas including hypertension control, diabetes care, immunizations, and maternal health. Sonal is deeply committed to advancing equitable, patient centered care and reducing disparities across diverse populations.
Prior to joining Boston Medical Center in 2022, Sonal held quality improvement leadership roles at Boston Children’s Hospital and community health centers, where she designed and led data-driven initiatives to improve chronic disease management, care coordination, and access to preventive services. Her work spans academic medical centers, safety net systems, and population health programs, with a strong focus on translating data into actionable improvement strategies. She has also led provider collaboratives and evaluated innovative tools such as care coordination workflows and patient engagement through digital outreach.
Sonal holds an MD in Pathology from Baroda Medical College, India and a Master of Public Health in Health Policy and Management. She is a Certified Professional in Healthcare Quality (CPHQ) and a Lean Six Sigma Green Belt. With more than 15 years of experience in clinical care, quality improvement, and population health, Sonal brings a comprehensive and practical perspective to advancing value-based care and improving health outcomes.
