Description

Amid a growing crisis in chronic disease and maternal health, healthcare must look beyond pharmaceuticals and address root causes, starting with nutrition. For many, a poor diet and chronic disease go hand-in-hand with food insecurity, limited access to culturally relevant nutrition, and other barriers to care—all of which impact dietary behaviors and health outcomes. Food is Health is a clinically proven, root-cause strategy that delivers against core health plan objectives—across Medicare, Medicaid, and commercial lines of business. This session presents an overview of key measures that highlight the full impact of a Food is Health program, including compelling evidence on cost reduction, clinical improvement (such as A1C reductions of up to 2.2 points), and whole-person outcomes. Beyond clinical data, you’ll hear members’ real-life experiences, highlighting the role of personalized, culturally relevant support in building trust and self-efficacy.

Attendees will gain a clear framework for embedding Food is Health into population health strategies that drive sustainable behavior change and elevate whole-person care.
