The OU Sooner Health Access Network (HAN) was one of the earliest community based complex care management models in the nation and has grown into Oklahoma’s first NCQA accredited complex care management program. Serving more than 80,000 Medicaid members—including individuals with complex medical, behavioral, and social needs—the OU Sooner HAN blends evidence-based models, interdisciplinary care, and person-centered engagement strategies.
