Older adults experiencing polypharmacy are at increased risk of a multitude of adverse health outcomes, including falls. There are plenty of evidence-based approaches that interdisciplinary care teams can use to address polypharmacy, but many have not been adopted consistently or sustainably. This talk will share successful approaches that teams serving older adults can leverage, based on the real experiences of a quality improvement learning collaborative with nursing homes, which reduced polypharmacy for older adults. Attendees will learn about available tools and frameworks, and the common approaches and challenges involved in implementing them.
