As health plans face rising utilization, workforce strain, and persistent quality gaps, Community Health Workers (CHWs) are emerging as a critical, cost effective extension of care teams. This session highlights Kaiser Permanente’s enterprise wide approach to scaling an evidence based CHW model across multiple regions, populations, and lines of business in partnership with IMPaCT Care. Speakers will share practical lessons on cohort selection, technology enabled infrastructure, workforce integration, and operational alignment with NCQA best practices to improve HEDIS performance, reduce utilization, and strengthen care team capacity.
