The transition of the Person-Centered Outcome (PCO) measures into the Healthcare Effectiveness Data and Information Set (HEDIS) is a critical step in improving care quality for Medicare Advantage Special Needs Plans (SNPs). This session will explore the structured implementation of PCO measures, including feasibility testing, data reporting processes and integration with existing SNP workflows. The discussion will highlight the role of early-adopter SNPs in refining these measures and overcoming barriers to data transfer and usability. Speakers will provide an in-depth review of how the PCO measures align with SNP Models of Care, sharing structured processes for goal tracking and data reporting that will support the integration of the PCO measures into existing workflows and HEDIS. The session will cover NCQA’s experience convening a learning collaborative of three SNPs to implement the PCO measures and report on performance data. Through this collaborative, NCQA and participating SNPs explored strategies to: • Adapt existing systems to collect and report measure data. • Identify and quantify the resources needed to build/modify data collection systems to track goal conversations. • Implement training and technical assistance to support SNP care managers Attendees will gain practical knowledge on measure implementation, lessons learned from early adopters, and broader implications for Medicare Advantage plans.
