Diabetes care is at an inflection point. Advances in diabetes technology, interoperable data, and clinical decision support have made proactive, technology-enabled, data-driven care increasingly achievable. Leading health systems are using these capabilities to identify people at risk earlier, personalize care, intervene to improve outcomes and reduce avoidable utilization. Yet realizing the full potential of these innovations will require more than technical advances or isolated success stories. 

This Quality Conversation will explore the policies, standards, evidence, and payment models needed to make data-enabled diabetes care sustainable, equitable, and broadly adopted. Speakers will discuss emerging priorities to protect patient rights, advance interoperability, support adoption, and integration, and expand equitable access to technology-enabled care.
