Addressing whole-person care requires strong connections across the health care system. In this session, we’ll explore Pennsylvania’s approach to connecting Pennsylvanians to resources to meet their social needs. Speakers will discuss the policy drivers at the Pennsylvania Department of Human Services, the role Health Information Exchanges (HIEs) play in improving overall health and wellness, and the essential community partnerships needed to build an effective state-wide social need platform to facilitate greater connection and communication between providers and organizations that serve shared populations.
